Left and Right Hand Numbness: Heart, Neck or Hand Nerve?

Quick answer

Left hand numbness is usually caused by a trapped nerve at the wrist, elbow or neck, exactly like the right hand. It becomes an emergency when it comes with chest pain or pressure, sweating and breathlessness (heart attack) or suddenly with weakness, facial droop or slurred speech (stroke) — call 123. Right hand numbness is more often carpal tunnel syndrome because it is the dominant hand for most people.

About your surgeon: Prof. Dr. Ibrahim Shaarawi

  • Professor of Orthopaedic Surgery, Faculty of Medicine, Ain Shams University.
  • Visiting Professor of Orthopaedic Surgery, Ohio, USA.
  • Certificate in complex fractures and trauma, University of Maryland, USA.
  • He has performed more than 4,536 nerve releases (carpal tunnel, ulnar nerve and Guyon’s canal) and more than 2,543 tendon releases (trigger finger, trigger thumb and De Quervain’s), alongside more than 6,735 targeted injections for hand nerves and tendons.
  • This includes more than 230 endoscopic carpal tunnel releases, and he has managed more than 1,100 hand cases without surgery.
  • Rated 4.9 and 5.0 on Google by about 300 patients.

He sees hand patients at his clinics in Nasr City, Mohandeseen and New Cairo (5th Settlement).

Patients often worry that left hand numbness means heart trouble — sometimes rightly. But in orthopaedic practice most persistent left or right hand numbness is a compressed nerve. This article helps you tell them apart, as part of our guide to hand numbness: causes and treatment.

When is left hand numbness an emergency?

With the numbness…It may meanWhat to do
Chest pain or pressure spreading to the left arm or jaw, sweating, nausea, breathlessnessHeart attackCall an ambulance (123) now
Sudden one-sided weakness or numbness with facial droop or slurred speechStrokeCall an ambulance (123) now
Severe neck pain after an injurySpinal injuryEmergency care, keep the neck still
Gradual finger numbness, worse at nightNerve entrapmentOrthopaedic assessment and nerve tests

Common causes of left hand numbness

  • Carpal tunnel syndrome: thumb, index and middle finger numbness, worse at night.
  • Ulnar nerve entrapment: little and ring finger numbness with elbow bending.
  • Cervical disc or arthritis: neck and shoulder pain radiating down the arm — the usual cause of “left hand and shoulder numbness”.
  • Sleeping on the left side and compressing the arm.
  • Diabetes and vitamin B12 deficiency (usually both hands).

Prof. Dr. Ibrahim Shaarawi explains two common causes of hand weakness and numbness (in Arabic).

Right hand numbness: why carpal tunnel is common

The right hand is dominant for most people and does more repetitive work — writing, phone use, housework, crafts — so carpal tunnel syndrome often appears there first and may later affect both hands. The other causes are the same: ulnar nerve, neck and systemic disease.

Key numbers to know

  • Carpal tunnel syndrome is the most common peripheral nerve entrapment worldwide (Padua et al., Lancet Neurology 2016).
  • Ulnar nerve entrapment at the elbow is the second most common entrapment neuropathy after carpal tunnel syndrome (Cochrane review, 2016).
  • In a Cochrane review of 14 randomised trials (1,231 patients, 84% women), surgery roughly doubled the chance of long-term improvement compared with splinting, and 44% of the splint group were later referred for surgery (Lusa et al., Cochrane 2024).
  • A steroid injection clearly improved symptoms at 10 weeks, but 73% of patients given the higher dose still had surgery within a year, versus 92% after placebo (Atroshi et al., Annals of Internal Medicine 2013).
CauseFirst-line treatmentWhen surgery is considered
Carpal tunnel (thumb, index, middle)Night splint and steroid injectionModerate to severe compression or thumb weakness
Ulnar nerve (little and ring fingers)Avoid elbow bending; night splintHand muscle weakness or moderate to severe compression
Cervical spine (numbness with neck and arm pain)Physiotherapy and medicationReferral to a spine surgeon if weakness progresses
Vitamin B12 deficiency or diabetes (hands and feet)Treat the cause and replace the deficiencyNo surgery; medical treatment
Heart or stroke (sudden numbness with chest pain or facial weakness)Emergency care immediately—
Table: treating hand numbness by cause

At Prof. Dr. Ibrahim Shaarawi’s clinics in Nasr City, Mohandeseen and New Cairo (5th Settlement) we examine the hand and review your nerve study at the same visit, confirming where and how badly the nerve is trapped before choosing a splint, injection or release.

Diagnosis and treatment

After emergencies are excluded, we rely on examination and nerve conduction studies, sometimes with a neck MRI. Treatment usually starts with a night splint, activity changes and treating the cause, moving to carpal tunnel release and hand nerve surgery or ulnar nerve entrapment when moderate or severe entrapment is confirmed. For numbness at night see hand numbness at night.

Frequently asked questions about left and right hand numbness

Is left hand numbness a sign of a heart attack?

It can be when it comes with chest pain or pressure, sweating, nausea or breathlessness — call an ambulance immediately. Numbness alone without these symptoms is usually a trapped nerve.

What causes left hand and shoulder numbness?

Usually a cervical disc or neck arthritis pressing on a nerve root, with neck and shoulder pain radiating down the arm.

What causes right hand numbness?

Most often carpal tunnel syndrome because it is the dominant hand, then ulnar nerve entrapment, neck problems and diabetes.

Is numbness in the fingers of the left hand serious?

Usually not; it points to a trapped nerve depending on which fingers are affected. It becomes serious if sudden, or with weakness, chest pain or speech difficulty.

When should I see a doctor?

Immediately with emergency signs, and within days if numbness lasts over two weeks, wakes you at night or comes with weakness.
